Abstract

현재 뇌졸중의호발 연령은 점점 낮아지고 있지만 뇌경색에 대한 이해나 인식은 보편화되지 않은 상황이다. 이 환자의 경우 증상 발생 즉시 내원하여 유로키나제를 사용한 혈관 시술을 하게 되어 효과적인 치료로 인한 긍정적인 결과의 한 예가 되었다. 이 환자의 입원시 의식불명의 상태였지만 입원 1주째에는 의식이 뚜렷하게 회복되었고 연하곤란도 점차 호전되어 정상적인 식이가 가능했다. 그러나 뇌졸중의 가장 큰 휴유증이라 할 수 있는 언어장애와 편마비가 남아 있어 환자 자신의 불편함의 물론 가족의 부담감이 많이 요구되는 상황이었다.
간호사로서 이 환자를 대할 때 가장 중점을 둔 것은 환자 자신에 대한 긍정적인 적응과 단시간에 호전되기는 어렵지만 끊임없는 훈련과 노력으로 편마비와 언어장애의 불편함을 극복할 수 있다는 자신감을 갖도록 하는 것이었다. 다행히 이 환자의 경우 입원기간동안 이루어진 일상생활동작 훈련을 통해 스스로 자가관리를 할 수 있다는 강한 의지와 자신감을 보여주었고 가족들이 언어훈련 및 신체손상 방지를 위해 실시한 의료진의 설명과 교육에 대해 환자와 가족이 모두 높은 관심과 인식수준을 나타내어 앞으로 좋은 예후가 기대된다.
부연하자면 뇌졸중의 가장 효과적인 치료는 예방에 있으므로 호발 연령을 대상으로 한 다양한 교육 프로그램의 개발 및 제공이 이루어져야 하고 조기 발견의 기회를 확대해 가는 것이 중요하리라 생각된다. 그리고 퇴원 후 추후관리 시스템 즉, 가정 간호의 활성화 방안을 모색해야 할 것이다.

As the 21st century approaches, one of the very important issue for nursing is the increased prevalence of chronic condition such as stroke. The stork is reported the highest rate of death in Korea, 1998. A stroke is always stressful for the family. Therefore, the preventive measures for stroke should be emphasized. And the health education is provided to stroke survivors and caregivers for their adjustment to the change related to stroke.

The positive self-concept and motivation are focused on early rehabilitation for overcoming disability. Fortunately, the client has strong will and supportive family for rehabilitation. So, we expect this case is improving.
