Abstract

|
본 연구는 재가 허약노인을 방문하는 보건소 방문간호사와 지역사회간호학에서 허약노인 방문실습을 마친 간호대학생이 인식하는 허약노인 대상자를 위해 간호대학생에게 필요한 시뮬레이션 교육 요구도가 어떠한지를 탐색하기 위해 포커스 그룹 면담을 하였다.
우리나라는 지역사회 통합돌봄(커뮤니티 케어)으로 인하여 방문간호에 대한 요구와 필요는 점점 증가할 것이다. 그 중 허약노인은 장기요양보험 등급외자로 재가에서 적절한 의료서비스 제공이 미흡하며 방문간호서비스도 제한적이며 보건소 간호사가 제공하는 ‘방문건강관리사업’은 노인의 장애를 막기 위해 방문간호사의 지속적이며 절대적인 간호대상이 되고 있다. 본 연구의 결과에 따른 허약노인에 대한 이해, 정신건강 중재, 치료적 관계 형성, 간호술기, 건강생활습관 증진을 위한 보건교육, 지역사회 자원연계, 방문간호사 보호 시뮬레이션 교육 요구를 반영하여 시뮬레이션 간호교육 운영에 적용한다면 추후 방문간호사의 핵심역량 증진에 기여할 수 있을 것이다.
이에 본 연구의 결과를 기반으로 시뮬레이션 교육의 장점을 활용하기 위해서 간단한 술기 위주의 시나리오보다는 실제와 유사한 복잡하고 다양한 상황의 허약노인 시뮬레이션 시나리오 개발과 시뮬레이션 학습의 효과에 대한 연구가 이루어질 필요가 있다. 더불어, 방문간호 경력 간호사의 허약노인 간호 역량개발을 위한 고급단계의 시나리오 개발과 활용도 필요하다.

Purpose: This study aimed to identify the educational needs of nurses and nursing students for the development of a simulation module of home visiting care for frail, elderly people.
Methods: Focus group interviews were conducted with 15 home visiting nurses working in public health centers and 14 nursing students who experienced home visiting from September 10 to October 10, 2018.
Results: Bloom’s taxonomy of learning objectives, namely, cognitive, affective, and psychomotor domains was used as a framework for data analysis. The defined educational needs for each domain were as follows: “understanding frail, elderly people” for the cognitive domain; “intervention for mental health” and “building a therapeutic relationship” for the affective domain; and “nursing skills”, “health education for healthy lifestyles”, “referral to the community resource connection”, “protection for visiting nurses” for the psychomotor domain.
Conclusion: Based on the findings of this study, a simulation module of home visiting care for frail, elderly people can be developed and used for nursing students and nurses to strengthen the capacity for home visiting care.
